The Children and Families Specialty Plan (CFSP) is a statewide Medicaid plan tailored to meet the unique needs of children and families navigating the child welfare system. Operated under the name Healthy Blue Care Together, this initiative is administered by Blue Cross NC in close collaboration with local agencies and communities.
- Statewide, Unified Coverage
Healthy Blue Care Together serves as a single, statewide plan responsible for managing healthcare services for its members. This approach simplifies the care process and ensures that families, caseworkers, caregivers, and providers can easily access the support and resources they need.
- Locally Informed and Community-Based
The plan is shaped by input from local Department of Social Services offices, families, and community stakeholders. With care managers embedded in local communities, services are tailored to address the specific needs of each region.
- Family-Focused and Prevention-Driven
The CFSP emphasizes preventive care and family preservation, with a strong focus on reducing foster care placements, supporting family reunification, and promoting long-term stability and well-being.
- Access to Comprehensive Services
Members benefit from timely access to a full spectrum of services—including physical health, behavioral health, pharmacy, long-term services and supports (LTSS), intellectual/developmental disability (I/DD) services, and other health-related needs.
- Coordinated, Whole-Person Care
Every member receives dedicated care management to ensure seamless coordination across service providers and treatment settings. The plan is designed to support smooth transitions, whether members are aging out of the system, experiencing a placement change, or transitioning out of Medicaid.

At Extended Hands Assisted Living & Home Care, we are proud to join forces with Obedience of Faith Corp, a nonprofit organization dedicated to empowering youth and young adults, in support of the Healthy Blue Care Together – Children and Families Specialty Plan (CFSP).
This vital program, established by the North Carolina Department of Health and Human Services and managed by Blue Cross NC, seeks to provide enhanced, coordinated healthcare and supportive services to children, youth, and families in the child welfare system.

Together, Extended Hands and Obedience of Faith Corp bring a powerful combination of healthcare, life skills training, case management, and transitional support services to communities across North Carolina.
Through this partnership, we aim to:
